Denise M. Dluzak (Guminski) age 71, of Lake in the Hills passed away on Friday, October 5, 2018 at Alden Estates of Barrington, with her loving family at her side. She was born on June 18, 1947 in Chicago, the daughter of Edward and Geraldine (Boyke) Guminski.

Denise is survived by her husband Charles Thomas Dluzak; children, David Dluzak and Michelle (Jeff Razniewski) Dluzak, her granddaughter Grace Dluzak; and brother Ronald (Sheryl) Guminski. She was preceded in death by her parents and her brother Edward Guminski.

Services were held privately. Memorials may be made in memory of Denise to the Alzheimer’s Association.
